Serving the Homewood community, Baptist Health Brookwood Hospital is a 595-bed acute-care hospital located in Birmingham, Alabama. The facility offers a full complement of services, including emergency care, cardiovascular care, bariatrics, orthopedics and rehabilitation, and surgical services. It also is one of the largest providers of psychiatric care in the state. The hospital has earned Primary Stroke Center certification from The Joint Commission, and its bariatric program has been accredited as a  Comprehensive Center by the Metabolic and Bariatric Surgery Accreditation and Quality Improvement Program (MBSAQIP). The Women’s Medical Center provides comprehensive women’s services through all stages of life, including gynecologic surgery, maternity care, and labor and delivery services. Additionally, the Level III Neonatal Intensive Care Unit (NICU) holds Advanced Certification in Perinatal Care from The Joint Commission.

Postition Summary

 

This role will enhance the patient experience by providing seamless navigation from pre-procedure scheduling and throughout the care continuum. Collaborates with all members of the interdisciplinary team to coordinate and consistently manage patient care by serving as a point of contact for patients and families. Systematically and continually perform the functions of assessing, planning, implementing, and evaluating care according to the nursing process.

Responsibilities

  • Coordinates patient care through the continuum by assisting with timely access to care, coordinating/facilitating appointments and services, and encouraging adherence to treatment plans, protocols, and follow up care.
  • Assesses patient current and future needs, provides patient access to resources (internal and external), and makes appropriate referrals as needed.
  • Provides education and information to the patient and family related to the healthcare system, multidisciplinary team member roles, plan of care, and available resources.
  • Establishes and maintains close collaboration with physicians and multidisciplinary team members to ensure seamless patient care and care coordination.
  • Tracks determined program metrics, and quality indicators, documents patient interactions and progression, and communicates navigation activities to physicians, care team, and leadership.
  • Collaborates with clinical team to obtain or develop patient and family educational materials and resources.
  • Demonstrates knowledge, skill, and coordination to provide nursing care and guidance to the patient from screening to post-operative care and follow up.
  • Demonstrates effective communication with patients, families, peers, members of the multidisciplinary healthcare team, and community organizations and resources.
  • Perform all other duties as assigned.
  • Maintains reasonably regular, punctual attendance consistent with Orlando Health policies, the ADA, FMLA, and other federal, state and local standards.
  • Maintains compliance with all Orlando Health policies and procedures.
  • Actively participates in multidisciplinary planning conferences.
  • Attends staff development in-services, department meetings, etc.
  • Establishes and maintains professional role boundaries with patients, families, and multidisciplinary care team.
  • Serves as advocate for patients and families.
  • Assumes responsibility for professional development and continuing education.

Qualifications

Education/Training

 

Graduate of an accredited school of Nursing. Associate's degree required. Bachelor's degree preferred.


Licensure/Certification

 

Current licensure as a registered nurse in the State of Alabama or Nurse Licensure Compact (NLC). Maintain current Basic Life Support (BLS) certification.

 

Experience

 

Three (3) years’ clinical experience in acute care. Orthopedic Experience preferred. Care Management/Coordinator experience preferred.
